Suppose you would like to take care of your forms while in our office, please feel free to do so. We ask that you arrive at least fifteen minutes before your visit to ensure you have plenty of time and allow us to keep your appointment and the appointments of others on the schedule.

As a courtesy we are happy to process your insurance forms and submit them to your insurance carrier. Regardless of insurance assistance, you are responsible for all fees related to services rendered.

Major credit cards are accepted, as well as personal checks. If you have no insurance to assist you with services, there is a 5% cash payment discount when you pay for your services with cash or check the day of your appointment.
